Objective: To evaluate smartphone addiction associated with auditory self-perception, daily hours and maximum volume with headphones among adolescents. This is an epidemiological, cross-sectional, analytical study, baseline of the ELCAS Project: “Longitudinal Study on Adolescent Behavior in Physical Activity and Health”. Methods: Students of both sexes, in the first year of high school from the state public school system (urban and rural) in the city of Montes Claros, Minas Gerais, Brazil, participated in the study. The collection was carried out in the classroom at pre-scheduled times. Besides the instrument regarding smartphone addiction, the Brazilian version of the Smartphone Addiction Inventory, the questionnaire included questions about self-perception of hearing, headphone usage in relation to time and volume of the device. Results: A total of 1,322 adolescents participated, 51.7% female, aged between 14 and 19 years. The prevalence of problematic smartphone users was 54.9%. There was an association between those considered smartphone dependent and female gender (RP: 1.244; CI: 1.124-1.376), those who reported difficulty understanding words (RP: 1.157; CI: 1.037-1.291) and considered their hearing average/poor (RP: 1.253; CI: 1.129-1.390), users of more than four hours per day (RP: 1.146; CI: 1.029-1.277) and those who use at maximum volume (RP: 1.378; CI: (1.251-1.518). Conclusion: The high prevalence reflects a worrying behavioral addiction. Further studies are urgently needed to elucidate the association between smartphone dependence and hearing damage.
