Disseminated cutaneous leishmaniasis: successful case report with miltefosine treatment

Objective: to report the case of a patient with disseminated cutaneous leishmaniasis caused by L. braziliensis, treated with miltefosine. Method: a case report study, with information obtained through interviews, photographic records and a review of medical records. Results: the male patient presented with an initial ulcerated lesion and subsequent dissemination of acne-like lesions in multiple regions of the body. The direct test for Leishmania sp. was negative, while the molecular test confirmed L. braziliensis. The treatment was well tolerated, with no adverse effects reported. The treatment consisted of administering miltefosine orally for 28 days, with monthly clinical follow-up. After 12 months, the lesions had completely healed without recurrence. Conclusion: The case demonstrates the efficacy of miltefosine in the treatment of disseminated cutaneous leishmaniasis, highlighting its potential as a first-choice therapeutic alternative. The absence of adverse effects and the maintenance of clinical response reinforce its viability, especially in immunocompetent patients, contributing to the advancement of knowledge about therapeutic strategies for this uncommon form of the disease.
